On March 27, 2023, an 18-year-old employee of Troyer Roofing & Coatings, a Missouri contractor, fell over 22 feet from a roof and died from injuries sustained in the fall. OSHA investigation revealed that the employer allowed employees to decide whether to use fall protection equipment and did not ensure its use, resulting in a willful violation. The company also failed to train employees on proper fall protection, forklift operations, face and eye protection, and hazard communication. The agency proposed penalties of $205,369 and cited the company for similar fall protection violations in 2015.
